Terms and Conditions 

Guest Acknowledgment & Agreement

I am the person whose name appears on this form and all information is correct to the best of knowledge. In addition, I understand that Elegance Recovery House LLC (is NOT a medical service, or medical provider). Elegance Recovery House is intended to provide a comfortable and private setting to assist with mental and physical recovery following outpatient surgery procedures. I understand that Elegance Recovery House is NOT a substitute for a medical care or hospitalization and , as such, I am electing to use the services of Elegance Recovery House as a matter of convenience and not for reasons related to medical care, I hereby acknowledge that I am of legal age and sound mental capacity and that I make this election free from duress or undue influences.

Financial Responsibility

A NON-REFUNDABLE deposit of $500 is due at the time of booking to reserve dates with balance due in full 14 days prior to check in. This fee reserves and secures your stay. Please be aware there is a 6% merchant processing surcharge for all transactions.

I agree that my liability for this bill is NOT waived in the event that the indicated person, company or association fails to pay and/or disputes the full amount of the charge or in the event of cancellation within 14 days.

I am the person that authorizes Elegance Recovery House to charge my credit card on for all agreed upon services. No refunds are given on any unused or cancelled stays or services. I Elegance Recovery House uses a secure payment method and does not sell information to any third party.

Check-In & Check-Out

Our check-in time is 4pm or directly after surgery discharge. Your guest can check in at that time. Check-out time is 11am. Please plan all post op appointments and flights accordingly.

If you need to check-in early or check-out late, please notify us immediately as there may be additional charges. Please plan your flights and check out pick-ups accordingly.

Once check out time approaches, I understand that I will need to leave the premises or I am subjected to a late checkout fee in the amount of an additional night stay.

I understand that check-in time closes @9:00 PM and I will NOT be granted access to the house after this time.

It is recommended that out of town guests check in the day before surgery. A guest cannot be removed from their bed prior to 11am. We must also sufficient time to fully sanitize between guests.

Your guest must follow all rules or they will be asked to leave the premises immediately.

Meals

I understand that my stay includes three meals per day at no additional cost. Elegance Recovery House does our best to accommodate dietary restrictions/Allergies. Please let us know at booking if you have any allergies or dietary restrictions including vegan, pescatarian, or vegetarian.

Elegance Recovery House does not cater to specific preferences outside of dietary restrictions and is unable to make changes to the menu if not noted prior to check-in.

Please feel free to order deliver such as Instacart or Uber Eats to the house if there are things on the menu you do not desire.

I agree to disclose all allergies prior to my arrival, and understand if the information is not given to Elegance Recovery House prior to my arrival, I will need to provide my own meals and changes to accommodate my meals will commence the following day.

Transportation

Transportation is only provided for Surgery Day, prescriptions, and 1 post-op appt pick up/drop offs. Additional rides should be communicated and arranged with staff and will incur an additional fee.

Additional rides are subject to Elegance Recovery House current daily transportation schedule. If there is a schedule conflict guests are open to Uber to locations.

I understand that Elegance Recovery House offers shared transportation service, and does not offer private car service. We do our best to plan accordingly with appointment times.

Please be advised if booking is impromptu/last minute or less that 72 hours there may be schedule conflicts in which transportation may or may not be included/provided.

Transportation is reserved solely for patient use.

Guests are financially responsible for parking, tolls, and any other expenses related to your surgery day, prescription pick up, and follow-up appointments.

If your arrival time is outside of the days and times we offer Transportation (Mon-Fri 9:00 AM - 3:00 PM. You are responsible to secure transportation to our facility.

In case of an emergency Elegance Recovery House will attempt to assist you as much as possible to the nearest urgent care or hospital (not that of your choice). Please be advised that you are responsible for the cost of an ambulance or any medical transport, medical bills, or any other medical costs concerning your emergency. Elegance Recovery House is not held liable or accountable, in any way, for any distress, medical bills, obligations, surgical issues, or death. Elegance Recovery House is NOT obligated to stay at the hospital/urgent care for any length of time or provide pick-up after release from hospital.

Due to the severity of hospitalization, urgent care or medical treatment, Elegance Recovery House reserves the right to deny reentry as guests are no longer a candidate for services. All guests are responsible for their own accommodations and transportation thereafter. Additional fees may apply if emergency transportation is needed.

Medical Risks and Responsibility

Elegance Recovery House is NOT a medical facility and is a service to help assist with daily living activities during my recovery. Please follow Doctors discharge instructions.

Further, I understand that my medical care, at all times, and in all circumstances remains the responsibility of myself and the surgeon who performed my surgery.

I understand that only my surgeon is authorized to write prescriptions for me and to determine what outpatient recovery care is appropriate for my particular recovery needs.

I understand that my surgeon is the only party with authority to order and perform post-operative medical treatment and he or she reserves the right, alone, to order my hospitalization, if that is the level of care I require.

I further understand that my surgeon is the only party authorized to determine the level of post-operative care needed following my procedure, including suggesting me to a non-medical facility.

*I understand that I may be unsteady on my feet following surgery and I agree to request assistance getting out of bed for the first 24-hrs following surgery.* In addition, while on pain medication, I agree to remain at Elegance Recovery House unless accompanied and evaluation from staff. If I choose to leave at any time during my stay, reentry is under the sole discretion of Elegance Recovery House.

In addition, I fully understand that it is MY responsibility to check with my surgeon, prior to my surgery date, for verification on which over-the-counter AND prescription medications I may take while recovering at Elegance Recovery House.

I agree to ONLY bring over-the-counter & prescription medications to Elegance Recovery House in which my plastic surgeon who performed the current procedure has verified or prescribed. I may safely restart on arrival to the facility. Any violation of this policy will be asked to leave the premises. Elegance Recovery House is not responsible for any harm or fatality that may result from drug abuse or additional narcotic use.

I understand that it is my responsibility to place ALL approved medications, BOTH prescription and over-the-counter, in my possession and keep them safe. Elegance Recovery House DOES NOT administer medication and is limited to reminding you about your medication. Elegance Recovery House is not responsible for medication not approved by surgeons.

Furthermore, I hereby agree to indemnify and hold harmless Elegance Recovery House and all of its owners, agents, officers, directors, employees, contractors and affiliates from and against any and all loss, liability, death, damage, cost, expense (including reasonable attorneys’ fees and disbursements and costs of investigation), judgment, charge, fine, interest, penalty or assessment resulting from, arising out of, or relating to, any act, omission or state of facts and any demand, action, suit, proceeding, claim, assessment, judgment or settlement or compromise, whether known or unknown, of any kind whosoever that I may sustain in connection with choosing not to comply with Elegance Recovery House Medical Risk and Responsibilities policy stated here.

**Medical Issues and Emergency are the sole responsibility of you and your surgeon during check in and check out. If you have a medical emergency after check-out please contact your surgeon immediately or call 911. Elegance Recovery House WILL NOT assist with questions, medical issues, emergencies, or complications after check-out. Please consult your surgeon.**

Medications & Prescriptions

**IT IS HIGHLY RECOMMENDED TO HAVE YOUR PRESCRIPTIONS FILLED PRIOR TO SURGERY* Elegance Recovery House IS NOT IN CONTROL OF PHARMACY FILL HOURS*

Rx pickups costs are absorbed by you.

Elegance Recovery House does not promote unhealthy eating habits and will NOT pick up any additional items/orders while picking up prescriptions. Guests are free to Instacart or purchase delivery services for any items or food needed.

We require you to bring cash to purchase your prescriptions. If for some reason you do not have cash or your method of payment fails and we must pay for your prescriptions, we have a 6% transaction fee that is added to the cost of your prescriptions.

Prescription drop off concludes at 5:00 PM and prescriptions will be taken to the pharmacy the following morning. The only exception is if the prescription were given on the day of surgery.

Elegance Recovery House Drivers are only permitted to go to Walgreens, CVS, and Osco to fill prescriptions. This is to avoid any delays with filling your prescriptions. You will have to provide your own transportation to an alternate pharmacy should you choose to decline the services at Walgreens, CVS, and Osco Pharmacy.

I understand it may take up to 8 hours to fill my prescriptions and Elegance Recovery House has no control over the given times at the pharmacy.

Again, I agree to ONLY bring over-the-counter & prescription medications to Elegance Recovery House , which my surgeon has verified I may safely restart on arrival to the facility.

Again, I hereby agree to indemnify and hold harmless Elegance Recovery House and all of its owners, agents, officers, directors, employees, contractors, and affiliates from and against any and all loss, liability, damage, cost, expense (including reasonable attorneys’ fees and disbursements and costs of investigation), judgment, charge, fine, interest, penalty or assessment resulting from, arising out of, or relating to, any act, omission or state of facts and any demand, action, suit, proceeding, claim, assessment, judgment or settlement or compromise, whether known or unknown, of any kind whosoever that I may sustain in connection with choosing not to comply with Elegance Recovery House medication policy stated here.

No Smoking Policy

I understand that Elegance Recovery House has a strict NO SMOKING policy, and I will be fined $300 if found smoking in my room and possible termination of reservation in which one would have to vacate the premises. This includes cigarettes, marijuana/weed, vape pens, and any other substance. I understand that smoking causes significant detriment to my body’s ability to heal and greatly increases my risk of post-op complications. I hereby agree NOT to smoke while at Elegance Recovery House. Elegance Recovery House has the right to remove you from the house in the event of smoking.

I also agree NOT to request the assistance of a Elegance Recovery House staff member and contractors, to assist me outside to smoke.

Should I decide to smoke and breach this contract, I am doing so at my own risk, and risk the possibility of being removed from the premises. I hereby agree to indemnify and hold harmless Elegance Recovery House and all of its owners, agents, officers, directors, employees, contractors, and affiliates from and against any and all loss, liability, damage, death, cost, expense (including reasonable attorneys’ fees and disbursements and costs of investigation), judgment, charge, fine, interest, penalty or assessment resulting from, arising out of, or relating to, any act, omission or state of facts and any demand, action, suit, proceeding, claim, assessment, judgment or settlement or compromise, whether known or unknown, of any kind whosoever that I may sustain in connection with choosing to smoke while at Elegance Recovery House.

I hereby agree to indemnify and hold harmless Elegance Recovery House and all of its owners, agents, officers, directors, employees, contractors, and affiliates from and against any and all loss, liability, damage, death, cost, expense (including reasonable attorneys’ fees and disbursements and costs of investigation), judgment, charge, fine, interest, penalty or assessment resulting from, arising out of, or relating to, any act, omission or state of facts and any demand, action, suit, proceeding, claim, assessment, judgment or settlement or compromise, whether known or unknown, of any kind whosoever that I may sustain in connection with choosing to have massages while at Elegance Recovery House.

Transportation

I hereby assume all risk and personal injury, sickness, death, damage, and expenses as a result of participation in transportation activities involved with Elegance Recovery House. Further, authorization and permission are hereby given to Elegance Recovery House to furnish any necessary transportation, and I hereby agree to hold harmless and indemnify Elegance Recovery House and all of its owners, agents, officers, directors, employees, contractors, and affiliates for any liability sustained by transportation services provided by Elegance Recovery House as a result of the negligent, Willful or intentional acts of Elegance Recovery House including expenses incurred attendant thereto and hereby give permission to hospitalize and hereby authorize medical treatment, including but not limited to emergency surgery or medical treatment and assume the responsibility of all medical bills if any. I understand that this is a legally binding release and consent.

I have carefully read this Release of Liability and Consent for Transportation and Medical Treatment policy, outlined here and fully understand its contents. Being aware of said contents, I sign of my own free will.

I understand that whenever I, am transported, that I hereby release and hold Elegance Recovery House and all of its owners, agents, officers, directors, employees, contractors, and affiliates harmless from any and all loss, liability, damage, death, cost, expense (including reasonable attorneys’ fees and disbursements and costs of investigation), judgment, charge, fine, interest, penalty or assessment resulting from, arising out of, or relating to, any act, omission or state of facts and any demand, action, suit, proceeding, claim, assessment, judgment or settlement or compromise, whether known or unknown, of any kind whatsoever that I, a family member or a friend, may sustain in connection with the services provided to me by Elegance Recovery House or any of the foregoing.

Reschedule

If you choose to reschedule, you must contact us 1 week PRIOR to your check-in date. There is NO extra charge for the first reschedule offense if contacted prior to 1 week. 2nd reschedule offense (for any reason) results in cancellation and guests will have to rebook with a fee of $500. You may use deposit and monies to transfer to another date under three conditions.

1. Your room is still available.

2. Surgery is rescheduled within 6 months.

3. Guests must pay the differential price of the going rate of room reserved (if 

Cancellations

All reservations must be cancelled via email or text 14 days prior to check-in. All monies are non-refundable for cancellations less than 14 days before check-in. This includes surgical center cancellations, missed flights, or personal cancellations. Cancellation policy is reassessed ONLY for terrorist attacks, current global pandemic (less than 6 months), political instability, or uncontrollable weather like volcanos or severe category 4 hurricanes.

If the surgical center cancels you for whatever reason on the day of surgery you forfeit not only your $500 deposit but all monies paid.

No changes can be made to your reservation once you check in. If you must depart early for any reason, you forfeit your deposit and all monies paid. If you choose to stay longer please contact the manager for availability.

If your full balance is not paid 2 weeks (14 days) prior to check-in your reservation will be canceled and you forfeit any monies paid. If we do not receive prior notice based on our cancellation policy you will forfeit your deposit or any monies paid.

You must submit all required forms, financial obligations, and provide flight information at least two weeks (14 days) prior to your arrival or we reserve the right to cancel your reservation and you forfeit all monies paid. All cancellations within guidelines will receive a credit to be used within 6 months.

We are unable to plan for your stay without your surgery and flight details. Please text, call, or email us surgery time (if you know it) and flight details.

Loss & Damage

Elegance Recovery House shall not be liable for any lost or damaged items during your stay. Elegance Recovery House recommends that you bring any personal property at your own risk. If you should leave any item (s) behind, you are responsible to pay the postage to return those items. Please note that any damages to furniture or property of the house will be charged to card on file and potential law enforcement involvement.

Recording

Elegance Recovery House does not allow any guest recordings, blogging/vlogging, social media, or any documentation of stay, the house, guests, staff, incidents, dissatisfaction or well-being of other guests during the entirety of your stay. You also give full consent to Elegance Recovery House to use your image, likeness on their website, YouTube, and all social media platforms. You release all rights to content, images, and recordings to Elegance Recovery House.

Code of Conduct

Elegance Recovery House is a home and we promote an environment that is conducive to healing. We require that you speak in a low tone while within the house and be considerate of other guests. We do not allow any profanity inside or on the premises of our facility. We require that you are respectful towards all guests and our staff at all times. Additionally you must use headphones while on FaceTime or any video call. We do not allow the use of speaker phones in order to preserve the peace of our other guests. Failure to follow this code of conduct will result in removal from our house and you will forfeit all the monies paid.

In consideration of ALL the foregoing clauses, I still elect to utilize the services of Elegance Recovery House . I agree to indemnify and hold harmless Elegance Recovery House and all of its owners, agents, officers, directors, employees, contractors and affiliates from and against any and all loss, liability, damage, death, neglect, cost, expense (including reasonable attorney’s fees and disbursements and costs of investigation), judgment, charge, fine, interest, penalty or assessment resulting from, arising out of, or relating to, any act, omission or state of facts and any demand, action, suit, proceeding, claim, assessment, judgment or settlement or compromise, whether known or unknown, of any kind whatsoever that I may sustain in connection with the services provided to me by Elegance Recovery House or any of the foregoing.

You agree that you shall not make any claims against Elegance Recovery House and specifically waive any and all rights you may have to assert any claim against Elegance Recovery House pursuant to this agreement for any damages and/or legal fees which may be sustained as a result of any liability asserted against or imposed upon you for the violation of any law, statute or regulation, or any civil liability or damages asserted against or sustained or incurred by you or any others, arising out of advice Elegance Recovery House renders to you.

You also agree to indemnify, hold harmless and defend Elegance Recovery House against any claim or liability which may be brought back against or imposed upon Elegance Recovery House, including but not limited to reasonable attorneys’ fees to defend against such, as a result of any action or lack of action by you or others as a result of the services rendered pursuant to this agreement, whether such claims are asserted by or against you by others, or against me by you or others.

Elegance Recovery House reserves the right to modify, update, limit or terminate these terms and conditions at any time.
